Activity Events Approval Form
This form is needed for each event to be approved and scheduled on calendar. Recurring Events must be completed per quarter. Special one time events, retreats, fundraisers, fundraisers, etc.. complete each time.

Activity Status Form

Event Promotion Request Form
Form to be utilized for every event.

Fundraiser Form
Form to be submitted for Fundraisers.

Maintenance

Ministry Progress Report
The Ministry Progress Report should be completed monthly.

Small Group Leader Report Form
A report for all Champions Small Group leaders to turn in after their meeting each month.

You will need Microsoft Word to view and edit the forms and documents.

Click on the document you would like to download, then select Save.
